  • As a Direct Pay Integrative Health practice, Performance Medicine is a fee-for-service business. We do not accept any insurance.

    Why?

    Performance Medicine is committed to providing our patients with the highest quality care without insurance company intervention. By using a fee-for-service system, we can avoid the restrictions and influence insurance companies use to streamline a patient’s care.

    We can spend more time directly with the patient, not with insurance companies (coding, EMR’s etc.) We work for you only.
     

    Payment Methods Accepted:

    • Cash

    • Credit or debit card

    • HSA (Health Savings Account)

    • Care Credit
       

    Each service and visit that we offer varies in cost. Please click here to contact your local Performance Medicine office to inquire about prices.

  • Step 1: Click here to print and fill out a new patient form.

    Step 2: Obtain a hormone panel lab work up from a lab of your choice or come into Performance Medicine to use Quest Labs.

    Step 3: After your lab work, you may schedule an appointment for a Hormone Consult.

    Step 4: After your Hormone Consult, your Provider will tell you when they would like to see you for a follow up visit. Please schedule your follow up visit when you checkout from your consult.

     

    Appointment openings are limited, and a spot may not be available during the right timeframe if you wait to schedule.

    When beginning Bio-Identical Hormone Replacement Therapy, there will be an adjustment period where your Provider will prescribe a dosage of Bio-Identical Hormones based on your preliminary lab report. To ensure your dosage is correct, your Provider will request follow up lab work.

    After your Provider is satisfied with your dosage, you will be expected to get blood work done every 3-6 months to monitor your hormone levels.

  • Yes! We use Quest and Path Group to process lab orders drawn in office.

    You can file insurance with our lab, but we do find not all insurances cover hormone lab testing,  

    OR

    You may pay out-of-pocket for blood work. For some people, this is a more affordable option than using insurance. Call your local Performance Medicine office to inquire on the out-of-pocket price for a hormone panel.

  • Depending on the service, follow up visits should be every 3-6 months. 


    For example:

    • ADHD follow-ups must be every 3 months

    • Hormone Replacement Therapy follow ups are every 3-6 months depending on their course of treatment.

    • Routine non-controlled medications follow ups are every 6 months.
       

    Your Provider will always inform you of when you should schedule your follow up appointment.
